1. A person needs to have a primary diagnosis of a Mental Illness.
  2. An Admission form needs to be filled out and documentation needs to be provided, showing that the individual is/has receiving(ed) services from a mental health professional (psychiatrist, psychologist, therapist, or caseworker) for their primary diagnosis of a mental illness.
  3. Membership card will be issued upon completion of admission policy/forms.

In order to complete the admission for membership process, prospective members will need to present the above-mentioned forms (completed) and meet with a staff member. Upon meeting with staff and completing all required paperwork (takes less than 10 minutes), you will be issued a free membership card.
